Clinical philosophy
Patients with four conditions usually have four specialists and no one reading all four letters. That gap is where an interaction hides, and it is most of what internal medicine is actually for.
She reconciles the medication list at every visit rather than once a year, out loud, with the box in front of her. It takes six minutes and it is the single highest-yield thing she does.
She is direct about uncertainty. If two specialists disagree about you she will say so, tell you which way she leans, and put the reasoning in the note so the next person can see it.
