What this job actually is
A new post, created because three separate pathways kept referring the same patients outward and losing them. You would sit inside the diabetes, cardiac rehabilitation and paediatric services rather than beside them, seeing patients in whichever building their clinician sits in.
Three days a week, one of which is remote for follow-ups and group work. The travel between clinics is inside the rostered day, not on top of it.

Day to day
What you would be doing
- Hold clinics at all four buildings on a published weekly pattern
- Take referrals directly from primary care, cardiology, paediatrics and behavioural health
- Run the eight-week group programme for newly diagnosed type 2 diabetes
- Advise colleagues on nutrition questions through the shared record
- Help shape the practice's approach to weight, which is currently inconsistent between clinics
What we need
What you would bring
- Registered Dietitian Nutritionist (RDN) with Oregon licensure
- Three years post-registration, including chronic disease management
- Experience of group facilitation as well as one-to-one work
- A driving licence, since this post covers four buildings
