The role
What this job actually is
This is a clinical post and not a dispensing one. You would run your own medication review clinics at Pearl District, take on the practice's polypharmacy work, and be the person colleagues ask before they add a ninth medicine to an eighty-year-old's list.
Deprescribing is treated as work rather than as a favour: the clinics are rostered, they are twenty-five minutes each, and the outcomes are reviewed quarterly with the primary care team.

Day to day
What you would be doing
- Run structured medication review clinics, including deprescribing where it is warranted
- Reconcile medicines after every hospital discharge that reaches the practice
- Advise on interactions and dosing through the shared record, in hours
- Maintain the practice formulary and lead the quarterly review of it
- Support the diabetes and anticoagulation pathways alongside their clinical leads
What we need
What you would bring
- PharmD with current Oregon pharmacist licence
- Board certification in ambulatory care (BCACP), or eligibility within eighteen months
- Oregon prescriptive authority under a collaborative practice agreement, or willingness to obtain it
- Two years in a clinical rather than dispensing setting
